このたび、本学の教育の改善を目的として、学部生（休学者は除く）を対象に、以下の要領で授業評価アンケートを実施しますので、ご協力をお願いいたします。

Ⅰ．実施期間：2019年12月16日（月）～2019年12月20日（金）

Ⅱ．対 象 者：学部生（休学者は除く）

Ⅲ．対象科目等：2019年度秋学期に開講されている科目（通年科目を含む）

　　　　　　　※卒業論文演習は除きます。

Ⅳ．実施方法：

　実施期間中の各授業内においてアンケート用紙を配付しますので、教員の指示に従い、必ずその場で記入・提出してください。

Ⅴ．その他：

　本アンケートは、本学の自己点検・評価活動の一環として、学生の皆さんの意見を今後の教育改善に結びつけることを目的に実施するもので、それ以外の目的には一切使用いたしません。アンケート全体の集計・分析結果と個別の授業に対する意見は、すべての成績が確定した後に、各教員に対してフィードバックする予定です。

　　　　　　　　　　　Course Evaluation Survey for the Fall Quarter 2019

　TUFS will be conducting a Course Evaluation Survey for courses taught in the Fall Quarter (including one-year courses) of AY 2019 for the purpose of improving the quality of education. (Research Seminars will be excluded.) We are asking all undergraduate students in the three schools, except for those on leave of absence (“Kyugaku”), to participate in this survey.

　The evaluation survey will be conducted from Monday, December 16 to Friday, December 20.

　You will receive the evaluation survey sheet from your instructor in class. Please follow the instruction, fill out the survey, and submit it during the class period.

　Your responses to this evaluation will be examined and used solely for the purpose of improving the quality of education. The results of the survey will be shown to the instructors after the final grades have been processed.
