この申請は、総合情報コラボレーションセンターが発行した パスワードを紛失または忘却した場合に再発行するものです。
注意事項/Notes
本申請をすることで、「総合情報コラボレーションセンター利用規程」の内容を理解し、 承認したものとします。
総合情報コラボレーションセンターが発行した初期パスワードを変更したい場合、 または、現在のパスワードを変更したい場合は、「パスワード変更ページ」(学内限定)をご利用下さい。
申請の理由を簡潔に書いて下さい。
なお、理由の内容によっては申請を受付できない場合もあります。

下の申請書をダウンロードし、必要事項を記入した上で、以下の場所へ提出して下さい。

交付書の発行/The issue of permits
申請の受付は毎週月曜日で締め、交付日はその週の金曜日になります。
交付書は申請した場所で受け取って下さい。
The weekly cutoff for submissions is Monday, and permits will be issued on Friday.
Please obtain your permit at the location where you submitted your application.
