[for TUFS students and staff] Information on booster (3rd dose) shots for COVID-19 vaccine at Tokyo Institute of Technology

To TUFS Students and Faculty Members

Tokyo Institute of Technology (Tokyo Tech) has decided to include TUFS students, staff and faculty members in the booster (3rd dose) shots (workplace vaccination) for COVID-19 vaccine. TUFS students, staff and faculty members who wish to be vaccinated at Tokyo Tech are requested to confirm the following information and apply for the vaccination by yourselves.

Thanks to the cooperation of the Tokyo Institute of Technology, the booster shot targets include people related to TUFS, and you must not post or tell others about this case of social media, including Instagram, LINE, Twitter and Facebook, or on a BBS, etc.

1. Eligible for vaccination

Those who satisfy both (1) and (2) below.

(1) Students, faculty and staff of TUFS.

 ①Students.
  ・Students enrolled at TUFS (including non-degree students, research students)

 ②Faculty members
 ・Faculty and staff affiliated with TUFS
(Regardless of whether it is full-time or part-time. Includes researchers, dispatched staff)

(2) At present, all of the following conditions must be met among those aged 18 years or older at the time of vaccination

① Persons who have been vaccinated at least six months after completing the first (first, second) vaccination with COVID-19 vaccine.
(The vaccine can be any Pfizer, Takeda/Moderna or AstraZeneca vaccine approved by the Japanese authorities)

② Those who have a vaccination voucher for booster shots (3rd dose) sent by the municipality
(Later submissions are not accepted).

※Those who have received other domestic unapproved vaccines abroad, except those listed above, cannot be vaccinated at the Tokyo Institute of Technology vaccination site. If you wish to be vaccinated, consult your municipality or other appropriate authorities. Please also see the COVID-19 Navi (Ministry of Health, Labor and Welfare https://v-sys.mhlw.go.jp/en/).

※The conditions for age and vaccination vouchers are set by the Ministry of Health, Labour and Welfare (MHLW) for the implementation of the workplace vaccination (booster shot)

5. Vaccination schedule.

Scheduled for the week of 13 June 2022

※The number of vaccinations will be determined after confirming the number of vaccinators in the first stage of application. Please adjust your schedule so that you can be vaccinated on whichever date is specified.

6. How to apply for reservations

Those wishing to receive booster shots at Tokyo Tech should apply and reserve an appointment as follows. The deadline for the first stage application is Monday 25 April at 16:00.

If you do not wish to receive booster shots at Tokyo Tech, you do not need to apply

Step 1: [First stage application] Fill in the application form to apply.

Those who have made a first-stage application must make an appointment

Step2. [Reservations] Make a reservation via the reservation system URL sent to you on completion of the first stage application.

(The vaccination voucher number for the booster shots (3rd dose) sent by the municipality is required at the time of booking the appointment.)

Please do not cancel appointment except in unavoidable circumstances, such as sudden illness on the day of the appointment, to prevent the vaccine being disposed of

※The URL of the booking system will be notified to the applicant as soon as the vaccination implementation date is fixed after the application deadline.

※In the application form, please select "related persons of TUFS"

※Do not post or tell others about the URL of the application form or reservation system on Instagram, LINE, Twitter, Facebook or any other social media or BBS.

7. What to bring on the day of vaccination

① "Vaccination Voucher" for the booster shots and a "Notification of Booster Shot for COVID-19 Vaccine (3rd dose)"

(In some municipalities, “Screening questionnaire with printed vaccination voucher")

※If you cannot bring your vaccination voucher on the day of vaccination, you will not be able to receive the vaccination.

②Evidence of the reservation (e.g. email confirming the reservation)

③Student or employee ID card (those without a student or employee ID card should bring an official ID card)

・For inquiries on COVID-19 vaccination, please contact the Tokyo Metropolitan Government COVID-19 Side Effects Consultation Centre (03-6258-5802, 24 hours a day, all days, Languages supported: Japanese, English, Chinese, Korean, Vietnamese, Tagalog, Nepali, Burmese (Myanmar), Thai, French, Spanish and Portuguese)
