（日本の高等学校のみ）調査書において、一部成績評価がない箇所がありますが、どうしたらよいですか。
Some Japanese High schools don’t mention grades on Investigation report. What should I do in that case?

  高等学校が作成している調査書ですので、作成された内容で受け付けます。

  留学している期間の調査書が書けないと言われた・・・との問い合わせがしばしばありますが、調査書は高等学校生徒指導要録をもとに作成されるものですので、高校の先生に、文部科学省の大学入学者選抜実施要項内の「調査書記入上の注意事項等について」の｛「各教科・科目等の学習の記録」の欄｝の項目に従って処理をするように依頼してください。

※ If you have studied abroad and need to mention the grade and study period on the Investigation report, ask your high school teacher to fill in the report by following the Admission Guideline provided by the Ministry of Education, Culture, Sports, Science and Technology.

（補足）
  文部科学省が通知している、「（高校生に係る）指導要録に記載する事項等」に記載のとおり、留学時の成績や在籍等にかかる証明書（またはその写し）は要録に添付されているはずです。

（Supplement Information）
　"The items to be mentioned on investigation report" which is notified by the MEXT and certificates and transcrips during the period of study abroad should be attached to the investigation report.
